Stabilization Specialist - Scattered Sites

Job Responsibilities:
Responsible for advocating for and assisting homeless and at-risk clients in obtaining and/or maintaining permanent housing. Provide assessment of stabilization and housing needs, and develop a plan to best meet housing and family stabilization goals. Conduct in-depth assessment utilizing the client’s strengths and challenges. Maintain contact with housed clients through home and office visits for a year. Assist clients with money management skills and applying for all potential benefits and resources. Serve as liaison to schools, medical providers and others to ensure best level of community integration. Contribute to the knowledge about successful outcomes through expanded/refined data collection and analysis. Coordinate rental assistance strategies and applications. Work with partnering agencies to ensure a comprehensive collaborative model and strategies are implemented. Perform other related duties as assigned from time to time.

Job Qualifications:
Minimum of a high school diploma or equivalent, with one to three years of experience in case management or related social services required. Associate’s degree in human services or related field preferred. Experience and knowledge of homeless and housing issues, and of housing subsidies. Willingness to conduct home visits in a variety of neighborhoods and to accompany clients to neighborhood services based on clients’ schedules required. Must be able to work and set appropriate boundaries with clients with mental health, substance abuse, and/or domestic abuse issues. Must have a valid driver’s license. Strong problem-solving and verbal and written communication skills required. Bilingual skills in Spanish highly preferred. Must be able to work sensitively and effectively with individuals of diverse educational, socio-economic and cultural backgrounds.
